The Child Care Subsidy Application will be due on Friday January 15th by 5pm in Snell 149.
OSU students will need to submit the following materials with your application:
MOST RECENT copy of your monthly child care bill
MOST RECENT copy of your OSU accounts information, including tuition, fees, scholarships and loans
MOST RECENT copy of your paycheck stub or verification of income for the household
and a COVER LETTER
